Fall sports participants will need to submit a completed physical form, Parent/Athlete Concussion Information Signature Page, and the PA Department of Health Sudden Cardiac Arrest Symptom Information Signature Page, before engaging in team tryouts or practices.
Fall sports participants AND marching unit participants will need to submit Activity Fee paperwork and payment before engaging in team tryouts or practices.
For High School students, forms will be collected at the Upper Moreland High School Main Office.
At the high school level, athletes will be able to hand in their forms and payments on August 7, 8, or 9 from 7:30 to 4:30. They will not be able to begin sports camps or tryouts on August 13 unless this paperwork has been handed in. For those who do not make the August 7, 8, or 9 deadline, their paperwork will be processed beginning at 7:30 on Monday, August 13.
At the High School level, marching unit participants may hand in their activity fee paperwork and payment on August 7, 8, or 9 from 7:30 to 4:30, or during the week of August 13 from 7:30 to 4:30.
Dates and processes for collection of Middle School forms will be provided to students upon their return to school. These forms will not be required before the beginning of the school year.
Students and families for whom the activity fee will represent a financial hardship will be able to request a reduction or waiver of the activity fee. This process will require a meeting with the high school or middle school principal, before engaging in team tryouts or practices.

Hard copies of forms may also be obtained at the High School Main Office during regular business hours - 7:30 AM - 3:30 PM.
There will be a mandatory high school meeting for all high school fall athletes on Monday, August 13, at 7:00 p.m. in the Upper Moreland High School Auditorium. This meeting will be followed by individual team meetings at the discretion of the fall coach. A parent or guardian will be required to attend with their son or daughter.
