School Year 2008- 2009

Dear Parents/ Guardians:

The School Health Act of Pennsylvania requires various immunizations and examinations for students enrolled in school.  Following is a list of required examinations and tests.  A chart listing the necessary immunizations for the various grade levels for the 2008- 2009 school year is provided on this link.

1. Medical examination is required upon original entry into school, in the sixth grade, and in the eleventh grade.  The medical examination must have been conducted after September 4, 2007.
2. Dental examination is required upon original entry into school, in the third grade, and in the seventh grade.  The dental examination must have been conducted after September 4, 2007.
3. Tuberculosis testing may be required for incoming students who are in the high-risk category.  During registration, you will be given a questionnaire to determine if your child is in a high risk category.

You are encouraged to have regular examinations performed by your family physician and dentist for complete and consistent care.

ATTENTION PARENTS/ GUARDIANS:

When you take your child for the physical examination, please ask your physician to include and record the following on the Private Physician Form:

1. Name of child and date of examination.
2. Record of height and weight.
3. Record of vision test.
4. Original and all booster immunization dates, which must include month, day, and year.
5. Scoliosis screening should be done for the sixth grade physical examination.

Enclosed are forms necessary for these examinations.  Please return completed forms to your child’s school/school nurse at the time of registration.  These examinations must have been conducted after September 4, 2007.
