Dear Parents/ Guardians:

Welcome to Upper Moreland School District!

If your child was born on or before September 30, 2003, he/ she is eligible for registration in our kindergarten program for the 2008-2009 school year.  Beginning with the 2008- 2009 school year, students enrolled in our kindergarten program will have the option of attending a full-day or half-day program.  So that adequate provisions can be made, it is necessary to begin the registration process now.

This year’s registration for all Upper Moreland kindergarten students will be held at the Upper Moreland Primary School, 3980 Orangemans Road Hatboro, PA, in the main office during the following times:

                        March 18, 2008          12:30 – 3:00 PM
                                                           5:30 – 8:00 PM

It is not necessary for you to bring your child to registration.  Please enter at the Primary School main entrance.  The outline below is intended to help you prepare for kindergarten registration:

1.  Complete the enclosed Registration Form and Health History.

2.  Please forward the immunization card (H-502.320) to your doctor immediately so that it will be returned to you before registration.  Please make sure your physician fills in the required immunizations, including the month, day, and year of each immunization. * Please speak to the nurse if vaccinations cannot be completed by the first day of Kindergarten.

3.  Bring the completed forms, proof of residence, and your child’s birth certificate.

4.  Proof of Residency:

• Proof of residence will consist of a Property Deed, Mortgage Statement or current lease agreement.  In addition, one or more of the following must be presented: Internal Revenue Statement, current Bank Statement, Driver's License, current Utility Bill or Cable Television Bill bearing the address on the Property Deed, Mortgage Statement or current Lease agreement.

• If the parent/ guardian is not the owner or lessee of the residence, a Multiple Occupancy Form must be completed and notarized.  Multiple Occupancy forms are available at the Student Services office, 2900 Terwood Road, Willow Grove.  If you have any questions regarding this matter, please contact the Student Services office at (215) 830-1515.  

• Even if you already have a child(ren) attending the Upper Moreland School District, proof of residency is required to be shown for each new student enrolled within the district.  Please be sure to provide your proof of residency for your child who will be attending the 2008-09 kindergarten program.

5.  Also included in the packet are medical and dental forms required by the Pennsylvania Department of Health.  Parents/ Guardians will need to have these forms completed and returned to the school nurse at registration, or as soon as possible thereafter, but no later than the opening of school.
