Your Child MUST Be Age 5 on or Before September 1, 2013 to Enter Kindergarten

 

Please download, print, complete, and bring all of the following items to Kindergarten Registration: If you do not have a printer, you may pick up the Kindergarten Registration Packet at the
UM Primary School, 3980 Orangemans Rd., Hatboro PA or at the
Administration Building at 2900 Terwood Rd. Willow Grove, PA 19090.

3. Obtain and present a copy of Child's Current Immunization Report from their Physician OR submit a Physician Signed Medical Exempt Form
   Immumization Form can be obtained here.
4. Proof of Residency:
   Proof of Residency will consist of a Property Deed, Mortgage Statement or current lease agreement.
   
   In addition; two (2) of the following must be presented: Internal Revenue Statement, current Bank Statement, current  cell phone, insurance, or current Utility Bill bearing the address on the Property Deed, Mortgage Statement or current Lease.
   
   If the parent is not the owner or lessee of the residence, download a Multiple Occupancy Form (page 3 of form is instructions) to be completed and notarized. Multiple Occupancy forms are also available at the Student Services Office, 2900 Terwood Road, Willow Grove. If you have any questions regarding this matter, please contact the Student Services office (215) 830-1515.
   
   If you already have a child(ren) attending the Upper Moreland School District, proof of residency is still required to be shown for each new student enrolled within the district. Please be sure to provide your proof of residency for your child who will be attending the 2012-2013 kindergarten program.
